Monroe County Community Health Improvement Plan
The hospitals of URMC, including Strong and Highland which serve Monroe County primarily, are committed to improving the health of the community. In Monroe County, all hospitals work collaboratively with each other, the Monroe County Department of Public Health, and several community agencies and programs to address pressing issues of concern for our community. This group, the Community Health Improvement Workgroup (CHIW), is coordinated through the Center for Community Health & Prevention and assures a review of data and community concerns around our most pressing issues, at least every three years, in the Community Health Needs Assessment (CHNA). The workgroup develops a Community Health Improvement Plan (CHIP) to address priority areas of need, and works with the Monroe County hospitals, health department and community partners to implement the plan, evaluate progress, identify barriers, and celebrate successes.
